BACKGROUND

Enhancing Hospital in the Home Service (HITH)

  • Initiated by the NSW Health Department 20 years ago, Hospital in the Home (HITH) allows patients with acute and post-acute conditions receive hospital-level care at home.

  • In 2016-17, more than 22,000 patients benefited from 50+ specialised programs for both adults and children, covering medical conditions across 81 diagnosis categories.

  • ~ 80% of cases involved complex care e.g. acute and post-acute treatments.

DESIGN PROCESS

The project started by doing comprehensive background and user research to evaluate the pragmatic quality of HITH services across Australia. User researchers involves inviting 40 stakeholders to participate in interview, survey and usability testing. Affinity diagram is used to analyse the major problems.

CHALLENGES

👩‍⚕️ Inconsistent Care Delivery

Due to rotation shifts, the nurse that visits patient varies. Nurse uses pen and paper to document patient’s condition and report to doctor. The lack of standardised handover leads nurse to spend time to reassess the patients condition instead of delivering continuous care, decreases efficiency.

🗂️ Fragmented Records Across Providers

Patients under HITH require care from multiple practitioners (nurses, GPs, specialists, radiologists, pharmacists), but patient records is stored individually in their own system. The fragmentation makes patient records difficult to consolidate, unable to provide coordinated care, delaying diagnosis and increasing risk of repeated tests and critical decisions.

🔍 Delays in Referral and Specialist Access

Access specialists requites GP referral, often limited by GP personal network. Due to specialist shortage, patient need to wait 3-9 months for an appointment. This leads to delayed decision,  treatment, worsen conditions. 

🤯 Lack of Patient Health Literacy

Many patients struggle to understand medical terms, medical side effects, as well ass medical conditions and treatment plans. As a result, they may fail to follow instruction, convey accurate information to new healthcare providers, compromising treatment outcomes.

  • e-Medical Record System is an online platform designed to enhance how health-related data is stored, accessed, and shared in Australia.

  • Centralised online platform where medical histories, treatment records, and consultations are securely recorded and made accessible in electronic format, with consent.

  • Promotes unprecedented data transparency, fostering effective collaboration across healthcare sectors. The result is a streamlined decision-making process for providers, improved patient care, and optimised health outcomes.

DESIGN SYSTEM

After sketching out the basic experience, I started to define the branding colour, typography (Font; Size; Line height; Spacing; Grey scale through a test and learn process. Despite colour choices from both existed and new designs both bring a sense of calmness, trust, security, the cold colour from existed design , i.e. Doder Blue with Grey in Gainsboro was unable to achieve a visual balance. Instead, the new colour choices help to maintain readability with harmony.

A Centralised Repository

The home page is designed to be informative in 2 ways. First, allow patients to update medical data to medical practitioners. Once it’s updated, it will be recorded and synchronised to doctor’s dashboard. This allows medical specialists to have access to patient’s information in timely manner. Second, it provides details about what medication patient is currently on, as well as upcoming appointment.

Enhance communication

The message feature allows patients and medical practitioners to stay connected and communicate with one-another, anywhere, anytime.

Maximise Utility

List of all the available appointments across different healthcare providers in Australia. Saving time and making the clinical experience easy and convenient, only by one click.

Data transparency

A record of patient’s medical history. Utilisng data transparency to provide timely access and accurate information for care, reduce duplication of tests and treatment and to enable an efficient and quality assure clinical practice.

Enrich Your Medical Knowledge

The medical library provides all the relevant medical information for user to access to broaden their understanding of medical knowledges. This includes: medical terminology, medicine detail, medical testing procedures and disease case studies.
